ICD-10-CM—the International Classification of Diseases, Tenth Revision, Clinical Modification—is used to code and classify medical diagnoses. ICD-10-CM is based on ICD-10, the system used to code and classify mortality data from death certificates. CDC’s National Center for Health Statistics (NCHS) is responsible for the clinical modification of ICD-10 for use in the United States.
